Instructions to Request Health Care Records
Under Texas law and HIPAA regulations, you have the right to inspect and receive a copy of your mental health and billing records.
To request access to or a copy of your records:
Submit a Written Request: You must submit a signed and dated written request to this practice via email at jennifer@jreyestherapy.com or through your secure client portal.
Processing Time: In accordance with Texas Health and Safety Code, our practice will respond to your request within fifteen (15) business days of receipt.
Fees: A reasonable, cost-based fee may be charged for copying, mailing, or preparing a summary of the records, as permitted by state law.
Texas Attorney General Consumer Complaint Notice
In addition to filing complaints with the licensing board, Texas consumers have the right to file complaints regarding unfair or deceptive business practices with the Office of the Attorney General (OAG) Consumer Protection Division.

Federal No Surprises Act (Good Faith Estimate)
Under Section 2799B-6 of the Public Health Service Act, health care providers are required to provide a "Good Faith Estimate" of expected charges to individuals who are uninsured or who are not using insurance (self-pay).
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, including psychotherapy sessions.
Your provider will give you a Good Faith Estimate in writing at least one (1) business day before your medical service or item. You can also ask your provider for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
